Did you connect the additional wire from the via marked on this picture to the Shuriken video board? It should be connected to the smaller pin header besides the FPGA on the Shuriken board, third pin from the Gamecube end. Without this wire you won't be able to enter the OSD, because the FPGA can't read the controller inputs and thus the OSD won't work.It works but there's no OSD or Sound.

You don't get any sound because by default GCVideo outputs a pure DVI signal to stay compatible with previous versions.

I'm not sure how that could not result in any results - searching for that string on digikey.com currently gives me 24 results in the IC->FPGA category. The one you want is this one.
